WAIVER

I understand that due to the way that pets interact with one another, minor injuries can occur even though the pets are carefully supervised at all times. If I am unreachable in the event of an emergency, whilst my pet(s) is/are in the care and custody of Pet Palz Pet Care Services, I hereby authorise Pet Palz Pet Care Services to seek immediate veterinary care for my pet.
I understand that all costs in connection with veterinary or other treatment shall be my responsibility. I hereby entrust the keys to my property and the care of my pet to Pet Palz Pet Care Services and I understand that Pet Palz Pet Care Services will not be liable for claims in relation to legal actions, loss, injury or damage of any nature, which I or my pet(s) may sustain or which may be caused in any way by my pet(s). I specifically, without limitation, agree to fully indemnify Pet Palz Pet Care Services for any and all such liability for claims in relation to legal actions, losses, injury or damage.
I certify that I have read and understood the terms and conditions of Pet Palz Pet Care Services, and confirm the truthfulness of the contents of the application form completed by me. Although Pet Palz Pet Care Services carefully screen all pets, occasionally we discover that this is not an appropriate partnership. Pet Palz Pet Care Services reserves the right to refuse to care for any pet, at any time.
